How is an orthopedic crown set on a tooth?

An orthopedic crown on a tooth is a prosthetic design that is put on the rest of the tooth after removing the affected part or in case of significant damage. The crown allows you to restore the shape, function and aesthetics of the tooth.

The procedure of installing an orthopedic crown usually takes place in several stages. First the tooth is prepared by removing the damaged part of it, then the tooth crown is created in our digital laboratory "Sa-Nath Dynasty" based on 3-D scanning. The crown can be made of various materials such as ceramic or zirconium dioxide. After manufacturing, the crown is placed on the tooth and, if necessary, adjusted and fixed on a permanent basis.

How is an orthopedic crown set on a tooth?

It is important to note that the new crown is installed exclusively on a healthy tooth, that is, before installing it must be diagnosed and treated if necessary. The process of installing an orthopedic crown is painless and is usually carried out using local anesthesia.

Steps of the orthopedic crown installation process:

Steps of the orthopedic crown installation process:
  • Collection of history. As with any other dental procedure, the installation of crowns begins with a professional examination and consultation with a specialist. Orthopedic dentist performs detailed diagnostics - X-ray, which allows to qualitatively study and evaluate the clinical picture of the patient, to choose the right ways to solve its problems. After receiving the diagnosed information, a further treatment plan is drawn up, which may include not only orthopaedic treatment but also therapeutic treatment. For the installation of the crown it is necessary to perform a number of preparatory works with the tooth.
  • Preparation for installation. Conduct of professional oral hygiene at the periodontist, hygienist or therapist to remove accumulated dental plaque and formed dental stone.
  • Rehabilitation of the oral cavity. Identification of problematic teeth: which are subject to therapeutic treatment, and which - removal. Treatment of caries, periodontological, surgical operations and endology of tooth canals or roots. You can not put a crown on a tooth in the mouth without prior sanitation!
  • Selection of the material of the future crown. Individual selection of the material from which the crown will be made for the patient. The crown material should be as aesthetically and functionally suitable to the patient as possible, taking into account his clinical situation and individual indications.
  • Color matching crown. The choice of color of the future orthopedic crown on the tooth is made using a special device - Vita scale (*Grisha, here is a reference to a separate article Color of teeth and Vita scale). Color according to the protocol should be determined exactly when a full professional teeth cleaning. Determining the color of the crown on the tooth is an important step in the process of fitting the crown, as it should match the rest of the teeth and give a natural look to the smile. There is a small nuance in this process: the tooth stub is responsible for the color of the enamel. If the tooth is unhealthy, dentin can change color and then the crown installed will be different from the other teeth in the smile. And vice versa: other unhealthy teeth nearby will change their color and will be significantly different from artificial orthopedic crown. For this reason it is very important to first treat all teeth, and then install an orthopedic crown.
  • Treatment of tooth under crown. Processing (dissection) of the tooth - giving the tooth the necessary shape, on which the future crown will "sit" firmly and firmly. It must be accurately fixed on the tooth, so each crown material has its own required tooth treatment, as one or another crown has its own individual thickness. Before installing the zirconium crown, the tooth needs to be prepared (treated) 1mm of tissue, for ceramic - from 0.5 mm to 1 mm. If the tooth is not insulated, local anesthesia is applied for its treatment before the installation of the crown for a comfortable procedure. The purpose of tooth preparation for the crown is to obtain at the exit of the cult base of the tooth (tooth stump), on which the orthopedic crown is firmly and reliably attached.
  • Making a crown. In order to individually produce a crown in our laboratory, 3-D scanning of the jaw is done using the 3-D scanner Sirona or separate required tooth. According to this scan in our dental laboratory technicians will produce individual orthopedic design. It is on the variety of the material of the crown depends on the duration of the manufacturing process. During the waiting period, a temporary plastic crown is installed to adapt and correct the patient.
  • Fitting and fixing the crown. After the crown is ready, it is necessary to make it fit before fixing. In the process of such a visit by a specialist, the density of the fit (shrinkage) of the crown on the tooth stump is assessed. In case the patient does not feel some discomfort, the crown is fixed to the tooth. It is also possible to temporarily fix the finished orthopedic crown in order to "test-drive" the crown for 3-4 weeks. If during this period of time no nuances are found on the comfort of operation of the crown, it is fixed to the patient on a permanent basis. This stage is the final in the process of installation of orthopaedic structure.

*It is important to remember that the procedure for inserting a crown on a tooth may differ depending on the individual characteristics of the patient and the condition of the teeth. Therefore, you should always discuss all the details of the procedure with your dentist.

A couple of theses about how to make an orthopedic crown:

  • Getting an Outfit. After the doctor has scanned the patient, he sends the scans to the case center, after which our technicians receive an outfit, where the type of work («technical task» in other words) is described in detail.
  • Design and milling. Then the design is created in a special computer program Exocad. Next step: put the crowns on milling depending on their material.
  • Synterisation (sintering of the crown). After milling, we place the sawn blanks of the crown in a special furnace for synterization.

  • Processing and painting. It is completely manual work, which is performed by a dental technician in the laboratory. The processing creates natural luster and natural structure of the tooth surface. Painting is similar to the process of painting by the artist. Special paints give the crown a natural flow, shades and shadows.
  • Inspection and delivery of work. The ultimate stage is the inspection on landing and delivery of the already finished work. Fitting after finishing the crown is mandatory. At this stage some adjustments can be made to bring the result to the ideal and maximum convenience for the patient.

Do not forget that orthopedic dentistry is not only about aesthetic prosthetics, but also about ensuring the chewing function of the tooth. And as for its aesthetic direction, thanks to modern techniques and materials, it is possible to restore the integrity of the tooth range, including individual selection of color, shape, texture, size and even position of the tooth in a row. Modern dentistry provides a wide range of material selection and a variety of ways of performing orthopedic crowns. A significant advantage is also that the clinic has its own laboratory for the manufacture of orthopedic structures. This provides the ability to choose the materials to be made, control each process of crown creation, and the ability to bring the result to 100% ideal.
